This form resides online where it can be opened by the volunteer.

After reader has read required statements he/she clicks on a button. If the user is already acquainted with this form and prefers to fill it in directly he/she is given that choice. If he or she is a novice he/she select may the questionnaire method.

After selecting the questionnaire method there are labeled blanks to be filled in or drop down lists to select from that automatically fill in the appropriate acronyms on the form.

There are buttons to navigate the questionnaire.

There are examples to explain what type of information is wanted.

There are explanations for sections that may be confusing.

Cells with red dots have pop up comments to display more information or regulations.

When he/she is done there is a button to print the form. A message comes up stating the required documents that must be filed with this form.

After printing, the path to the user's "My Documents" folder is discovered and the form is renamed and saved automatically for the user.

Instead of handwritten forms everything is very readable and correctly filled in.
